Voltaren: Tablet: Adults: The recommended initial daily dose is 100-150 mg. In milder cases, as well as for long-term therapy, 75-100 mg daily is usually sufficient.
The total daily dosage should be divided into 2-3 doses. To suppress nocturnal pain and morning stiffness, treatment with tablets during the day can be supplemented by the administration of a suppository at bedtime (up to a maximum daily dose of 150 mg).
In primary dysmenorrhoea, the daily dosage should be individually adjusted and is generally 50-150 mg. Initially, a dose of 50-100 mg should be given and, if necessary, raised in the course of several menstrual cycles up to a maximum of 200 mg/day. Treatment should be started upon appearance of the first symptoms and, depending on the symptomatology, continued for a few days.
Gastroresistant tablets should be swallowed whole with liquid preferably before meals.
Children ≥1 year: 0.5-2 mg/kg body weight daily, in 2-3 divided doses, depending on the severity of the disorder. For treatment of juvenile rheumatoid arthritis, the daily dosage can be raised up to a maximum of 3 mg/kg in divided doses.
Voltaren tablets of 50 mg are not recommended for use in children.
Ampoule: Adults: Voltaren ampoules should not be given for >2 days; if necessary, treatment can be continued with Voltaren tablets or suppositories.
IM Injection: The following directions for IM injection must be followed in order to avoid damage to a nerve or other tissue at the injection site.
The dosage is generally one 75-mg amp daily, given by deep intragluteal injection into the upper outer quadrant. In severe case (eg, colic), the daily dose can exceptionally be increased to 2 injections of 75 mg, separated by an interval of a few hours (1 into each buttock). Alternatively, 1 amp of 75 mg administered as soon as possible, followed by suppositories up to 100 mg on the same day if required. The total dosage should not exceed 175 mg on the 1st day.
IV Infusion: Voltaren must not be given as an IV bolus injection.
Immediately before starting an IV infusion, Voltaren must be diluted with saline 0.9% or glucose 5% infusion solution buffered with sodium bicarbonate. (See Cautions for Usage.)
Two alternative dosage regimens of Voltaren are recommended.
For the treatment of moderate to severe postoperative pain, 75 mg should be infused continuously over a period of 30 min to 2 hrs. If necessary, treatment may be repeated after a few hours, but the dosage should not exceed 150 mg within any period of 24 hrs.
For the prevention of postoperative pain, a loading dose of 25-50 mg should be infused after surgery over 15 min to 1 hr, followed by continuous infusion of about 5 mg/hr up to a maximum daily dosage of 150 mg.

Treatment of migraine attacks with Voltaren suppositories should be started with a dose of 100 mg at the first signs of an impending attack. Additional suppositories up to 100 mg may be taken on the same day if required. Should the patient require further therapy on the following days, the maximum daily dosage should be limited to 150 mg in divided doses.
Children: 0.5-2 mg/kg body weight daily, in 2-3 divided doses, depending on the severity of the disorder. For treatment of juvenile rheumatoid arthritis, the daily dosage can be raised up to a maximum of 3 mg/kg body weight, given in divided doses.
Voltaren SR: As a general recommendation, the dose should be individually adjusted. Adverse effects may be minimised by using the lowest effective dose for the shortest duration necessary to control symptoms (see Precautions).
Adults: The usual daily dose of Voltaren prolonged release tablet is 100 mg, i.e. one 100 mg prolonged release tablet. In milder cases and for long-term therapy, one 75 mg or 100 mg prolonged release tablet per day is normally sufficient. If symptoms are most pronounced at night or in the morning, the tablets should preferably be taken in the evening.
The maximum recommended daily dose is 100 mg.
The prolonged release tablets should be swallowed whole with liquid, preferably with meals.
Special dosage instructions: Paediatric patients (below 18 years of age): Voltaren 75 mg and 100 mg prolonged release tablets are not suitable for children and adolescents.
Elderly patients (aged 65 or above): No adjustment of the starting dose is generally required for elderly patients. However, caution is indicated on basic medical grounds, especially for frail elderly patients or those with a low body weight (see Warnings and Precautions).
Established cardiovascular disease or significant cardiovascular risk factors: Treatment with Voltaren is generally not recommended in patients with established cardiovascular disease or uncontrolled hypertension. If needed, patients with established cardiovascular disease, uncontrolled hypertension or significant risk factors for cardiovascular disease should be treated with Voltaren only after careful consideration, and only at doses of up to 100 mg daily if treated for more than 4 weeks (see Warnings and Precautions).
Renal impairment: Voltaren is contraindicated in patients with renal failure (GFR <15 mL/min/1.73 m2) (see Contraindications).
No specific studies have been carried out in patients with renal impairment, therefore, no specific dose adjustment recommendations can be made. Caution is advised when administering Voltaren to patients with renal dysfunction (see Warnings and Precautions).
Hepatic impairment: Voltaren is contraindicated in patients with hepatic failure (see Contraindications).
No specific studies have been carried out in patients with hepatic impairment, therefore, no specific dose adjustment recommendations can be made. Caution is advised when administering Voltaren to patients with mild to moderate hepatic impairment (see Warnings and Precautions).
